Keuchhusten

Whooping cough is a highly infecting disease caused by the bacterium Bordetella pertussis, more rarely caused by Bordetella parapertussis. After an unspecific initial stage, it usually runs over several weeks. On the initial stage with a cold-cough, which is called the catarrhale stage, the stadium-like convulsive tract is followed by typical staccato-like cough attacks. In infants, the cough attacks can manifest themselves untypically as respiratory standstill and thus are life-threatening. Finally, cough attacks gradually decrease in number and severity in the stage of decrement. A causative therapy is only possible at the initial stage. A generally recommended effective vaccination exists for prophylaxis. In Germany so far, there was a reporting obligation only in the case of death. Suspicion and disease of pertussis, as well as labordiagnostic proof, have become mandatory in Germany with the amendment of the Infection Protection Act of 29 March 2013. The epidemic law of Austria prescribes the notification obligation in case of illness and death. Keuchhusten ist eine durch das Bakterium Bordetella pertussis, seltener durch Bordetella parapertussis ausgelöste hochansteckende Infektionskrankheit. Nach einem unspezifischen Anfangsstadium verläuft sie regelhaft über mehrere Wochen. Auf das Anfangsstadium mit erkältungsartigem Husten, das Stadium catarrhale genannt wird, folgen im Stadium convulsivum anfallsartig typische stakkatoartige Hustenattacken. Bei Säuglingen können sich die Hustenanfälle untypisch als Atemstillstände äußern und somit lebensbedrohlich verlaufen. Schließlich nehmen die Hustenattacken an Zahl und Schwere im Stadium decrementi allmählich ab. Eine ursächliche Therapie ist nur im Anfangsstadium möglich. Zur Prophylaxe existiert eine allgemein empfohlene wirksame Impfung. In Deutschland bestand bisher eine Meldepflicht nur im Todesfall. Verdacht auf und Erkrankung an Keuchhusten sowie der labordiagnostische Nachweis sind mit der Änderung des Infektionsschutzgesetzes vom 29. März 2013 in Deutschland meldepflichtig geworden. Das Epidemiegesetz Österreichs schreibt die Anzeigepflicht im Erkrankungs- und Todesfall vor.
