Beriberi

Beriberi is a disease caused by the deficiency of vitamin B1 whose name comes from the Sinhala Beri which means "I can not", emphasizing with this term the marked fatigue that the patients of this deficiency show. The disease mainly affects the nervous and cardiovascular systems, so that the typical clinical picture is presented with anorexia, general malaise, weakness and paresthesias in the legs. It can also present with edema and palpitations. Classically, two presentations are described in adults and one in children or newborns. In adults, a "dry beriberi" is described if neurological symptoms or "wet beriberi" predominate if cardiovascular symptoms predominate. It is usually caused by insufficient diets, unbalanced or kept for too long. It became endemic in some regions highly dependent on rice when its mechanical husking became widespread. It is precisely there where the main deposits of thiamine reside in the rice grain. El beriberi es una enfermedad causada por la deficiencia de vitamina B1 cuyo nombre proviene del cingalés beri que significa «no puedo», destacando con dicho término la fatiga marcada que muestran los enfermos de esta deficiencia. La enfermedad afecta principalmente los sistemas nervioso y cardiovascular, de forma tal que el cuadro clínico típico se presenta con anorexia, malestar general, debilidad y parestesias en las piernas. Puede presentarse también con edema y palpitaciones. Clásicamente se describen dos presentaciones en adultos y una en niños o recién nacidos. En los adultos se describe un «beriberi seco», si predominan los síntomas neurológicos o un «beriberi húmedo» si predominan los síntomas cardiovasculares. Generalmente es causada por dietas insuficientes, desbalanceadas o conservadas por demasiado tiempo. Se hizo endémica en algunas regiones altamente dependientes del arroz cuando se generalizó su descascarillado mecánico. Es precisamente allí donde radican los principales depósitos de tiamina en el grano de arroz.
