Lactase

Lactase is an enzyme that catalyzes the hydrolysis of lactose into glucose and galactose. It is part of the intestinal secretion of young mammals and is essential for the digestion of milk. In adults who stop drinking milk, the production of this enzyme may decrease and cause difficulty in digesting milk or lactose intolerance. Inability to digest lactose can be easily suspected in adults or adolescents who develop gastrointestinal symptoms after drinking milk. It can be confirmed by acidity or reducing substances in the faeces after milk ingestion and absence of symptoms after lactose-free milk. The laboratory test used in clinical practice for the diagnosis of lactose intolerance is the lactose tolerance test, which consists in monitoring blood glucose after an oral dose of lactose. The test is considered positive if blood glucose measurements do not show an increase of 18 mg / dL between the initial fasting glycemia and the consecutive glucose levels performed at 20, 40 and 60 minutes. Nos adultos que deixam de tomar leite, a produção dessa enzima pode diminuir e causar dificuldade para digerir o leite ou intolerância à lactose. A incapacidade de digerir a lactose pode ser facilmente suspeitada em adultos ou adolescentes que desenvolvem sintomas gastrointestinais após a ingestão de leite. Pode ser confirmado pela pesquisa de acidez ou substâncias redutoras nas fezes após ingestão do leite e pela ausência de sintomas após a ingestão de leite isento de lactose. O teste laboratorial utilizado na prática clínica para o diagnóstico de intolerância à lactose é o teste de tolerância à lactose que consiste em monitorar a glicose sanguínea após uma dose oral de lactose. O teste é considerado positivo se as medidas de glicemia não demonstrarem uma elevação de 18 mg/dL entre a glicemia de jejum inicial e as glicemias consecutivas realizadas 20, 40 e 60 minutos.
