Heterophile Antikörper können bei jedem Immunoassay eine signifikante Interferenz verursachen. Die Anwesenheit eines heterophilen Antikörpers ist durch eine breite Reaktivität mit Antikörpern anderer Tierarten gekennzeichnet. Solche Antikörper werden üblicherweise als menschliche Anti-Tier-Antikörper bezeichnet. Für diese Kategorie gehören menschliche Anti-Maus-Antikörper. Sie können sowohl falsch positive als auch falsch negative Ergebnisse erzeugen. Sogenannte "Sandwich" Immunoassays sind besonders anfällig für diese Störung. Heterophile Antikörper können also falsche Positive oder falsche Negative geben. Sowohl das Erkennen als auch die Abschreckung dieser Interferenz ist in der klinischen Medizin schwierig. Eine Möglichkeit besteht darin, den Test mit einem anderen Testtyp zu wiederholen.

For example, in rheumatic fever, antibodies against group A streptococcal cell walls can also react with human heart tissues. These are considered heterophile antibodies. In clinical diagnosis, the heterophile antibody test specifically refers to a rapid test for antibodies produced against the Epstein-Barr virus, the causative agent of infectious mononucleosis. Heterophile antibodies can cause significant interference in any immunoassay. The presence of a heterophile antibody is characterized by broad reactivity with antibodies of other animal species. Such antibodies are commonly referred to as human anti-animal antibodies. Human anti-mouse antibodies belong to this category. They can create both false positive and false negative results. So-called 'sandwich' immunoassays are particularly susceptible to this interference. Heterophile antibodies may thus give false positives or false negatives. Both detecting and deterring this interference is difficult in clinical medicine. One option is to repeat the test using a different type of assay.
