Radioisotopenrenographie
Radioisotopenrenographie ist eine Form der Nierenbildgebung mit Radioisotopen. Die beiden am häufigsten verwendeten radiomarkierten pharmazeutischen Mittel sind Tc99m-MAG3 und Tc99m-DTPA. Einige andere radiomarkierte Arzneimittel sind EC und 131-Iod-markiertes OIH. MAG3 ist bei weitem ein besserer diagnostischer Wirkstoff als Tc-99m-DTPA, insbesondere bei Neugeborenen, Patienten mit eingeschränkter Funktion und Patienten mit vermuteter Obstruktion. Die MAG3-Clearance ist in hohem Maße mit dem effektiven Nierenplasmafluss korreliert und die MAG3-Clearance kann als eigenständige Nierenfunktion eingesetzt werden. Nach intravenöser Verabreichung werden etwa 40-50% des MAG3 im Blut durch die proximalen Tubuli mit jedem Durchgang durch die Nieren extrahiert; Die proximalen Tubuli schneiden dann den MAG3 in das röhrenförmige Lumen. DTPA ist das zweithäufigste gebrauchte Nierenradiopharmazeutikum in den USA, vor allem weil es das am wenigsten teuer ist. Tc-99m-DTPA wird durch den Glomerulus filtriert und kann zur Messung der glomerulären Filtrationsrate verwendet werden. Der Extraktionsanteil von DTPA beträgt etwa 20%, weniger als die Hälfte von MAG3. Allerdings ist EC bevorzugt, wenn das Serum Creatinin hoch ist.
