Glomerular Basement Membrane A thin layer of a single layer of 350mm thickness bounded by the epithelial cells and endothelial cells of the capillary loop of the glomeruli. Type VI collagen, laminin, and heparan sulfate proteoglycan secreted from the epithelium and endothelial cells are the main constituents, supporting tissues of the capillary system of the glomeruli, and at the same time, filtering apparatus for plasma components. For the latter, GBM is acting as a size barrier and a charge barrier. If the size barrier is lost due to the basement membrane obstruction, then large molecular weight serum protein such as globulin or fibrinogen is excreted into the diuretic and the charge barrier disappears, and a small amount of serum protein such as albumin is excreted into the urine. GBM is liable to become an obstacle to the nephritis development because the immune complexes in the serum are deposited or the anti-GBM antibody binds. 사구체기저막 사구체모세혈관계제(係蹄)의 상피세포와 내피세포를 경계로 하는 두께 350mm의 1층의 얇은 막. 상피, 내피세포로부터 분비되는 Ⅵ형 콜라겐, 라민닌, 헤파란황산 프로테오글리칸이 주요구성 성분이며, 사구체의 모세혈관계제의 지지조직이며, 동시에 혈장성분의 여과장치로도 되어 있다. 특히 후자에 대해서는 GBM은 size barrier과 charge barrier로서의 구실을 다하고 있다. 즉 기저막의 장애로 인해 size barrier가 잃어지면 글로블린이나 피브리노겐과 같은 대분자량의 혈청단백이뇨속으로 배설되어 charge barrier가 소실되면 알부민 등의 소분자량의 혈청단백이 대량으로 뇨중으로 배설된다. GBM은 혈청중의 면역볼합체가 침착하거나, 항 GBM항체가 결합하여 신염발증의 장애가 되기 쉽다.
