Es un polisacárido que no atraviesa la barrera hematoencefálica y requiere un largo tiempo de metabolismo en el manitol. Como es largo en la sangre, actúa como una sustancia osmóticamente activa. Por lo tanto, se usa como una solución de diurético osmótico o plasma sanguíneo y se puede usar para el tratamiento de la insuficiencia renal aguda y el edema cerebral. La insuficiencia cardíaca no se usa como diurético porque aumenta el volumen de plasma y aumenta la carga cardíaca y empeora los síntomas. 만니톨 체내에서 장시간의 대사를 요하며 혈액 뇌관문을 통과하지 않은 다당류로 혈중에 길게 존재하기 때문에 삼투압 활성물질로서의 작용이 있다. 이때문에 삼투압이뇨제 또는 혈장대용액으로서 사용되며, 급성신부전, 뇌부종의 치료에 이용되는 경우가 있다. 심부전에서는 오히려 혈장량을 증가시켜서 심부하를 증대하고 증상을 악화시키는 가능성이 있기 때문에 이뇨제로는 이용하지 않는다.
