Se define como el cambio a un compuesto deseado in vivo después de la administración del profármaco. Aunque es un fármaco útil, está químicamente modificado para ser clínicamente utilizable porque tiene propiedades que no son adecuadas para efectos secundarios, estabilidad, solubilidad, absorción de agua, tiempo de acción y similares. Por ejemplo, un medicamento antiinflamatorio no esteroideo llamado profármaco, que es un fármaco antiinflamatorio no esteroideo, se ha desarrollado como una sustancia inactiva que se absorbe en el tracto digestivo y luego se metaboliza en el hígado, convirtiéndose así en una sustancia activa. Es esperado. 프로드러그 투여후에 생체내에서 목적으로 하는 화합물로 변화되는 것으로 정의된다. 유용한 약물임에도 불구하고 부작용, 안정성, 용해성, 흡수성, 작용시간등에서 적합치 않는 성질을 가지고 있는 것에 화학적 수식을 가해서 임상사용 가능하게 한 것이다. 예를 들면 위장장애가 많은 비스테로이드 항염증약인 프로드러그가 개발되고 있는데, 불활성 물질로서 소화관에서 흡수되고 그 후 간장등에서 대사되어서 활성형의 물질로 변화해서 항염증 작용을 발휘하는 것으로 위장장애의 경감이 기대되고 있다.
