Antes que se descobrisse a cura da doença, os pacientes eram enviados a colônias isoladas, chamadas leprosários. Lá, cada um tinha que tomar conta de si, pois não havia muita ajuda externa. Eram lugares cruéis, sem lei e com pouca proteção, onde os pacientes tinham que levar adiante suas vidas. Roubo, violência e abusos eram comuns. Leprosários famosos e duradouros situaram-se em Kalaupapa, Chacachacare, The Basin e Spinalonga. Diversas colônias localizavam-se em ilhas, de modo a melhor segregar os doentes do resto da população. No Brasil um dos maiores leprosários foi o Pirapitingui, localizado entre Itu e Sorocaba, no estado de São Paulo.
