वैद्यकीय इतिहास
एखाद्या रुग्णाच्या वैद्यकीय इतिहासाचे किंवा प्रकरण इतिहासाची माहिती एखाद्या विशिष्ट डॉक्टरांद्वारे, एखाद्या रुग्णाने किंवा इतर व्यक्ती ज्यांना माहित असते आणि योग्य माहिती देऊ शकतात अशा काही विशिष्ट गोष्टी विचारून माहिती मिळविण्याचे उद्दीष्ट निदान निर्माण करण्यासाठी उपयुक्त आहे आणि रुग्णाला वैद्यकीय मदत देणे. वैद्यकीय कर्मचा-यांवर थेट तपासणी करून तपासलेले रुग्ण किंवा रुग्णाने ओळखलेल्या वैद्यकीयदृष्ट्या संबंधित तक्रारी रुग्णाशी निगडीत असलेल्या रुग्णांना क्लिनिकल लक्षणांच्या विरूद्ध लक्षण म्हणून असे संबोधले जाते. बर्याच आरोग्याशी होणार्या परिणामामुळे काही प्रकारच्या इतिहासावर परिणाम होईल. वैद्यकीय इतिहास त्यांच्या सखोलता आणि फोकसमध्ये बदलतात. उदाहरणार्थ, एखाद्या रुग्णवाहिकेचे वैद्यकीय अधिकारी विशेषत: त्याच्या इतिहासाला महत्त्वाच्या तपशीलांसह मर्यादित करते, उदा. नाव, तक्रारी सादर करण्याचे इतिहास, एलर्जी, इत्यादी. याउलट, एक मनोरोग इतिहास बर्याचवेळा आणि खोलीत आहे, रुग्णाच्या आयुष्याबद्दल जितके तपशील उपयुक्त आहेत मानसिक आजारासाठी व्यवस्थापन योजना तयार करणे
