पिनासिस ऍनीमिया हा मेगालोब्लास्टिक अॅनेमियाच्या मोठ्या कुटुंबातील एक प्रकार आहे. एक मार्ग अपायकारक अशक्तपणा विकसित होऊ शकतो जठरासंबंधी पॅरिअल पेशींचा तोटा म्हणजे काही भाग म्हणजे आंतरिक घटकांच्या विरघळणीसाठी, इलियममधील व्हिटॅमिन बी 12 च्या नंतरच्या शोषणासाठी आवश्यक असलेले प्रोटीन. साधारणपणे एट्रोपिक जठराची सूतिका बसून, जठराची पॅरिएटल पेशींचा स्वयंप्रतिकार नाश हा आंतरिक घटकांचा अभाव आहे. सामान्य आहारातील शरीरातील आंत शरीरातील शोषून घेणे शरीराच्या घटकांवर अवलंबून असते, त्यामुळे आंतरिक घटकांचा तोटा विटामिन बी 12 च्या कमतरतेकडे जातो. बी 12 च्या कमतरतेमुळे मेगॅलोबलास्टिक ऍनिमिया सूचित करण्यासाठी कधीकधी 'अपायकारक ऍनेमिया' हा शब्द चुकीचा वापर केला जातो, परंतु त्याचा उचित वापर एट्रॉफिक जठराची सूज, पॅरिअल सेल तोटा आणि आंतरिक घटकांचा अभाव यामुळे होतो. प्रौढ बी 12 च्या कमतरतेचे सर्वात सामान्य कारण व्हिटॅमिन बी 12 शोषण्याची क्षमता नष्ट होणे व्हिटॅमिन बी 12 ची प्रतिकारशक्ती कमी होण्यामागची कारणे किंवा गॅसटिक ऍसिडचे उत्पादन कमी करण्याच्या इतर अनेक अटींमुळे होऊ शकते, जे बी 12 च्या आहारामध्ये भाग घेते.

Pernicious anemia is one of many types of the larger family of megaloblastic anemias. One way pernicious anemia can develop is by loss of gastric parietal cells, which are responsible, in part, for the secretion of intrinsic factor, a protein essential for subsequent absorption of vitamin B12 in the ileum. Usually seated in an atrophic gastritis, the autoimmune destruction of gastric parietal cells leads to a lack of intrinsic factor. Since the absorption from the gut of normal dietary amounts of vitamin B12 is dependent on intrinsic factor, the loss of intrinsic factor leads to vitamin B12 deficiency. While the term 'pernicious anemia' is sometimes also incorrectly used to indicate megaloblastic anemia due to any cause of B12 deficiency, its proper usage refers to that caused by atrophic gastritis, parietal cell loss, and lack of intrinsic factor only. The loss of ability to absorb vitamin B12 is the most common cause of adult B12 deficiency. Impaired absorption of vitamin B12 may be due to a loss of intrinsic factor or to a number of other conditions that decrease production of gastric acid, which also plays a part in the absorption of B12 from foods.
