Kwestionariusz jest instrumentem badawczym składającym się z szeregu pytań i innych pytań w celu zebrania informacji od respondentów. Jako typ ankiety, kwestionariusze mają również wiele podobnych problemów związanych z budową pytań i sformułowaniami, które istnieją w innych typach sondaży.
