Além de seu papel na transfusão de sangue, o sistema de grupo sanguíneo Rh, especificamente, o antígeno D, é usado para determinar o risco de doença hemolítica do recém-nascido, pois a prevenção é a melhor abordagem para o manejo dessa condição. Como parte do pré-natal, um exame de sangue pode ser usado para descobrir o tipo de sangue de um feto. Se o antígeno Rh está faltando, o sangue é chamado de Rh-negativo. Se o antígeno estiver presente, ele é chamado de Rh-positivo. Quando a mãe é Rh-negativa e o pai é Rh-positivo, o feto pode herdar o fator Rh do pai. Isso torna o feto Rh-positivo também. Problemas podem surgir quando o sangue do feto tem o fator Rh e o sangue da mãe não. Uma mãe que é Rh-negativa pode desenvolver anticorpos contra um bebê Rh-positivo.
