Rh kan grubu sistemi, mevcut insan kan grubu sistemlerinden otuz üçten biridir. ABO'dan sonra en önemli kan grubu sistemi. Günümüzde Rh kan grubu sistemi, 50 adet tanımlanmış kan grubu antijenlerinden oluşur; bunların arasında D, C, c, E ve e beş antijen en önemlisidir. Sık rastlanan terimler Rh faktörü, Rh pozitif ve Rh negatif, sadece D antijenine karşılık gelir. Kan transfüzyonundaki rolünün yanı sıra, Rh kan grubu sistemi, özellikle D antijeni, önleme bu durumun tedavisine en iyi yaklaşım olduğu için yenidoğanın hemolitik hastalığı riskini belirlemek için kullanılır. Doğum öncesi bakımın bir parçası olarak, bir fetüsün kan türünü bulmak için kan testi yapılabilir. Rh antijeni yoksa, kan Rh-negatif olarak adlandırılır. Eğer antijen mevcutsa, buna Rh-pozitif denir. Annenin Rh-negatif ve babanın Rh-pozitif olması fetüs Rh faktörünü babadan devralabilir. Bu da fetüsü Rh-pozitif yapar. Fetüsün kanı Rh faktörüne sahipken annenin kanında sorunlar ortaya çıkabilir. Rh negatif olan bir anne Rh-pozitif bir bebeğe karşı antikor geliştirebilir.
