它是一种不通过血脑屏障并且需要在甘露醇中长时间代谢的多糖。由于它在血液中很长，所以它起到渗透活性物质的作用。 因此，它可用作渗透性利尿剂或血浆溶液，可用于治疗急性肾功能衰竭和脑水肿。 心力衰竭不被用作利尿剂，因为它会增加血浆量，增加心脏负担并使症状恶化。 만니톨 체내에서 장시간의 대사를 요하며 혈액 뇌관문을 통과하지 않은 다당류로 혈중에 길게 존재하기 때문에 삼투압 활성물질로서의 작용이 있다. 이때문에 삼투압이뇨제 또는 혈장대용액으로서 사용되며, 급성신부전, 뇌부종의 치료에 이용되는 경우가 있다. 심부전에서는 오히려 혈장량을 증가시켜서 심부하를 증대하고 증상을 악화시키는 가능성이 있기 때문에 이뇨제로는 이용하지 않는다.
