Cordotomie
Cordotomie ist ein chirurgisches Verfahren, das ausgewählte schmerzbildende Traktate im Rückenmark deaktiviert, um Schmerzen und Temperaturwahrnehmung zu erreichen. Dieses Verfahren wird häufig bei Patienten mit schweren Schmerzen aufgrund von Krebs oder anderen Krankheiten, für die es derzeit keine Heilung. Die anterolaterale Cordotomie ist wirksam für die Entlastung von einseitigen, somatischen Schmerzen, während bilaterale Cordotomien für viszerale oder bilaterale Schmerzen erforderlich sind. Die Cordotomie wird in der Regel perkutan mit der fluoroskopischen Führung durchgeführt, während der Patient unter örtlicher Betäubung steht. Eine offene Cordotomie, die eine Laminektomie erfordert, ist für Patienten mit schlechten medizinischen Bedingungen oft riskant, kann aber erforderlich sein, wenn eine perkutane Cordotomie nicht möglich ist oder ein Versuch versagt hat. Cordotomie wird jetzt ausschließlich für Schmerzen durch Krebs verwendet, wo die Behandlung auf Stufe 3 der Weltgesundheitsorganisation Schmerzleiter sich als unwirksam erwiesen hat. Cordotomie ist besonders für Schmerzen durch Asbest-ähnliche Krebsarten wie Pleura und Peritoneal Mesotheliom angezeigt. Obwohl die Cordotomie bei der Linderung der Schmerzen wirksam ist, ist die Wirkung meist vorübergehend. Dysästhesie entwickelt sich nach den meisten Cordotomien.
