El río se origina en las montañas de Acarai y fluye hacia el norte por aproximadamente 724 km entre Guyana y Suriname, vaciando en el Océano Atlántico cerca de Corriverton, Guyana y Nieuw Nickerie, Suriname. Un servicio de transbordador opera entre estas dos ciudades. La frontera guyanese-surinamesa es la orilla del río guyanés, pero Guyana disputas esto, basado en la doctrina Thalweg. El tribunal que definió la frontera marítima entre Suriname y Guyana en 2007 confirmó la soberanía surinamesa sobre todo el ancho del río Courantyne.
