Síndrome da insuficiência respiratória aguda
A "síndrome do desconforto respiratório agudo", anteriormente conhecida como "síndrome do desconforto respiratório", "síndrome do desconforto respiratório do adulto", ou "pulmão de choque", é uma condição médica grave e com risco de vida caracterizada por inflamação generalizada nos pulmões. Embora possa ser desencadeada por uma infecção respiratória primária, como pneumonia, é mais freqüentemente uma seqüela de sepse ou trauma significativo. O ARDS é uma doença do parênquima pulmonar que leva a uma troca de gases prejudicada. Está associada a liberação de citocinas pulmonares, barreiras endoteliais prejudicadas, perda de surfactante, acumulação de fluido nos espaços aéreos distantes e alterações fibróticas. A taxa de mortalidade varia amplamente com base na gravidade da doença e na idade do paciente, com uma mortalidade média em algum lugar entre 20% e 50%. O acrônimo ARDS anteriormente significava "síndrome do desconforto respiratório do adulto" para diferenciá-lo da "síndrome do desconforto respiratório infantil", que ocorre em bebês prematuros. No entanto, como este tipo de edema pulmonar também ocorre em crianças, o SDRA mudou gradualmente para significar "aguda" em vez de "adulto". As diferenças da síndrome infantil típica permanecem.
