An epulis is a hemispherical or mushroom-shaped structure, which mostly sits on the gingival margin. Synonymous is the focal fibrous hyperplasia. It is a granulation tissue formation of different cell structures. It is the result of a reaction of the tissue to a chronic inflammation, which has mostly arisen in a gingival sheath. An epulis can also be produced by a longer-lasting mechanical stimulus. In most cases, the removal of the tissue must be combined with the extraction of the tooth. The epulis also behaves clinically as a tumor, and may spontaneously resemble itself, but also tends to recur. An epulis on the gums is a classic example of a so-called tumor-like lesion or a pseudotumor. This granulomatous pseudotumor shows histologically a large number of giant cells of the type osteoclasts and is therefore also called giant cell pulis or epulis gigantocellularis. If the localization is not on the gingiva and has the same macroscopy and histology, it is called submucosal fibroma or irritable fibroma.

Eine Epulis ist ein halbkugeliges oder pilzförmiges Gebilde, das meist dem Zahnfleischrand aufsitzt. Synonym ist die Fokale fibröse Hyperplasie. Es handelt sich um eine Granulationsgewebebildung unterschiedlicher Zellstruktur. Es entsteht als Reaktion des Gewebes auf eine chronische Entzündung, die meist in einer Zahnfleischtasche entstanden ist. Auch durch einen länger andauernden mechanischen Reiz kann eine Epulis entstehen. In den meisten Fällen muss die Entfernung des Gewebes mit der Extraktion des Zahnes kombiniert werden. Weiterhin verhält sich die Epulis klinisch wie eine Geschwulst und kann sich spontan zurückbilden, neigt aber auch zu Rezidiven. Eine Epulis am Zahnfleisch ist ein klassisches Beispiel für eine sogenannte tumorartige Läsion oder einen Pseudotumor. Dieser granulomatöse Pseudotumor zeigt histologisch eine Vielzahl von Riesenzellen vom Typ Osteoklasten und heißt deshalb auch Riesenzellepulis oder Epulis gigantocellularis. Wenn die Lokalisation nicht an der Gingiva besteht und eine gleiche Makroskopie und Histologie aufweist, nennt man ihn submuköses Fibrom oder Reizfibrom.
