Un cuestionario es un instrumento de investigación que consta de una serie de preguntas y otras indicaciones con el propósito de reunir información de los encuestados. Aunque a menudo se diseñan para el análisis estadístico de las respuestas, este no es siempre el caso. El cuestionario fue inventado por Sir Francis Galton. Los cuestionarios tienen ventajas sobre otros tipos de encuestas, ya que son baratos, no requieren tanto esfuerzo del encuestador como encuestas verbales o telefónicas, ya menudo tienen respuestas estandarizadas que facilitan la compilación de datos. Sin embargo, estas respuestas estandarizadas pueden frustrar a los usuarios. Los cuestionarios también están muy limitados por el hecho de que los encuestados deben ser capaces de leer las preguntas y responder a ellas.
