L'anémie pernicieuse est l'un des nombreux types de la plus grande famille d'anémies mégaloblastiques. L'anémie pernicieuse à sens unique peut se développer est la perte de cellules pariétales gastriques, qui sont responsables, en partie, de la sécrétion du facteur intrinsèque, une protéine essentielle à l'absorption ultérieure de la vitamine B12 dans l'iléon. Généralement assis dans une gastrite atrophique, la destruction auto-immune des cellules pariétales gastriques entraîne un manque de facteur intrinsèque. Étant donné que l'absorption par l'intestin des quantités alimentaires normales de vitamine B12 dépend du facteur intrinsèque, la perte de facteur intrinsèque entraîne une carence en vitamine B12. Bien que le terme «anémie pernicieuse» soit parfois mal utilisé pour indiquer une anémie mégaloblastique due à une cause quelconque de carence en B12, son utilisation correcte se réfère à celle causée par la gastrite atrophique, la perte de cellules pariétales et le manque de facteur intrinsèque seulement. La perte de capacité d'absorption de la vitamine B12 est la cause la plus fréquente de carence en B12 chez l'adulte. L'absorption altérée de la vitamine B12 peut être due à une perte de facteur intrinsèque ou à un certain nombre d'autres conditions qui diminuent la production d'acide gastrique, qui joue également un rôle dans l'absorption de B12 des aliments.
