La renographie des radio-isotopes est une forme d'imagerie rénale impliquant des radio-isotopes. Les deux agents pharmaceutiques radiomarqués les plus courants utilisés sont Tc99m-MAG3 et Tc99m-DTPA. Certains autres produits pharmaceutiques radiomarqués sont EC et 131 OIH marqués au iode. Le MAG3 est de loin un meilleur agent de diagnostic que Tc-99m-DTPA, en particulier chez les nouveau-nés, les patients atteints d'une fonction altérée et les patients présentant une suspicion d'obstruction. La clairance MAG3 est fortement corrélée avec le flux plasmatique rénal efficace et la clairance MAG3 peut être utilisée comme mesure indépendante de la fonction rénale. Après administration intraveineuse, environ 40 à 50% du MAG3 dans le sang est extrait par les tubules proximaux avec chaque passage dans les reins; Les tubules proximaux sécrètent ensuite le MAG3 dans la lumière tubulaire. La DTPA est la deuxième radiopharmaceutique rénale la plus utilisée aux États-Unis, principalement parce qu'elle est la moins chère. Tc-99m-DTPA est filtré par le glomérule et peut être utilisé pour mesurer le taux de filtration glomérulaire. La fraction d'extraction de DTPA est d'environ 20%, soit moins de la moitié de celle de MAG3. Cependant, EC est préféré lorsque la créatinine sérique est élevée.
