マンニトール体内で長時間の代謝を要し、血液脳関門を通過していない多糖類で血中に長く存在するため、浸透圧活性物質としての作用がある。 このため、浸透圧利尿薬または血漿の溶液として使用され、急性腎不全、脳浮腫の治療に用いられる場合がある。 心不全ではむしろヒョルジャンリャンを増加させて芯負荷を増大して、症状を悪化させる可能性があるため、利尿剤としては使用しない。 만니톨 체내에서 장시간의 대사를 요하며 혈액 뇌관문을 통과하지 않은 다당류로 혈중에 길게 존재하기 때문에 삼투압 활성물질로서의 작용이 있다. 이때문에 삼투압이뇨제 또는 혈장대용액으로서 사용되며, 급성신부전, 뇌부종의 치료에 이용되는 경우가 있다. 심부전에서는 오히려 혈장량을 증가시켜서 심부하를 증대하고 증상을 악화시키는 가능성이 있기 때문에 이뇨제로는 이용하지 않는다.
