Pierwotnie patriarcha był człowiekiem, który sprawował autokratyczny autorytet jako familijczycy paterów nad rozbudowaną rodziną. System takiej reguły rodzin starszych mężczyzn określany jest mianem patriarchatu. Jest to greckie słowo, związek πατριά (patria), "rodowod, potomstwo", esp. przez ojca (wywodzący się z słowa πατήρ - patēr znaczy "ojciec") i ἄρχων (archon) rozumieniu "lider", "szef", "władca", "król" itp. Abrahama, Izaaka i Jakuba jako trzy patriarchowie ludu izraelskiego, a okres ich życia nazywany jest wiekiem patriarchalnym. Początkowo nabrał znaczenia religijnego w wersji Septuaginta Biblii. Dzisiaj słowo nabrało konkretnych znaczeń kościelnych. W szczególności najwyżsi rangą biskupi w prawosławie wschodnim, prawosławie wschodni, kościół katolicki (powyżej arcybiskupa arcybiskupa i prymasa), a także kościół wschodni w Asyrii nazywają się patriarchami (w niektórych przypadkach także papieżami).
