Północne krańce Sumatry graniczy z Wyspami Andamańskimi, natomiast w dolnej części wschodniej są wyspy Bangka i Belitung, Cieśnina Karimata i Morze Java. Góry Bukit Barisan, które zawierają kilka aktywnych wulkanów, tworzą kręgosłup wyspy, a na północno-wschodnich stronach znajdują się równiny nizinne z bagnami, mangrowe i złożone systemy rzeczne.
