The erythrasma is a harmless disease of the skin, caused by bacteria of the type Corynebacterium minutissimum. Predisposing factors are strong sweat formation or local hyperhidrosis, obesity, diabetes mellitus or immunosuppression. Preferred locations here are in particular intertriginous areas such as armpits, groins and other large skin folds, since this leads to sweat formation and disturbance of the acid balance of the skin. It can be recognized by a large, well-defined reddening of the skin, which later becomes scaly. Normally, there is no itching. The erythrasma is diagnosed on the basis of the clinical picture or with the aid of the UV light of a wood lamp, which is coral-red by porphyrin production of the exciters. Therapy of erythrasma consists of strict hygiene, the use of acidic soaps for the stabilization of the skin milieu and the local application of imidazole or fusidic acid-containing ointments. In severe cases, the systemic use of erythromycin is also possible. Generally, the affected skin should be kept dry.

Das Erythrasma ist eine harmlose Erkrankung der Haut, die von Bakterien des Typs Corynebacterium minutissimum verursacht wird. Prädisponierende Faktoren sind starke Schweißbildung bzw. lokale Hyperhidrosis, Adipositas, Diabetes mellitus oder Immunsuppression. Bevorzugte Lokalisationen sind dabei vor allem intertriginöse Areale wie Achseln, Leisten und andere große Hautfalten, da es hier verstärkt zu Schweißbildung und Störung des Säurehaushaltes der Haut kommt. Man erkennt es an einer großen, gut abgegrenzten Rötung der Haut, die später schuppig wird. Normalerweise gibt es keinen Juckreiz. Diagnostiziert wird das Erythrasma anhand des klinischen Bildes oder mit Hilfe des UV-Lichtes einer Wood-Lampe, wobei es durch Porphyrinproduktion der Erreger korallenrot leuchtet. Die Therapie des Erythrasmas besteht in strikter Hygiene, der Benutzung von sauren Seifen zur Stabilisierung des Hautmilieus sowie dem lokalen Aufbringen von Imidazol- oder Fusidinsäure-haltigen Salben. Bei schweren Verläufen ist auch der systemische Einsatz von Erythromycin möglich. Generell sollten die betroffenen Hautareale trocken gehalten werden.
